Parking Permits for Persons with Severe Disabilities

The DMV DOES NOT issue parking permits. If you are a resident of the Village of Fairport and wish to apply for or renew your parking permit please follow the instructions below:

  • Complete Part I of the Application
  • Part II must be completed by and signed by your Physician
  • Submit your application and a copy of your driver's license or non-driver's ID to the Clerk-Treasurer's Office. Form may be presented to or mailed to the Village of Fairport Clerk-Treasurer's Office at 31 S. Main Street Fairport, NY 14450

Permamnent Permits (BLUE) must be updated every 3 years.
